Exito Program Case Manager: Congreso de Latinos Unidos, Inc.

Congreso de Latinos Unidos, Inc.is currently seeking a Case Manager for their Exito Program on Education and Workforce Services division (EWS). The ideal candidate will engages the client (and her or his family as needed), building and nurturing relationships with the family and developing an understanding of each family member’s passions, strengths, resources and challenges. The Exito CM is embedded in Congreso’s Out of School Time programming at Edison High School. The CM carries a caseload of students who are demonstrating the early warning indicators of dropping out of school. The CM will serve as mentor and coach, creating and following-up on the initial client’s individualized learning plan, academic counseling support and connecting all family members to additional resources as needed.

Essential Responsibilities

Provides initial client orientation and conducts Comprehensive Assessments for student and other family members and then creates an action plan based on the outcome of the assessment.

Assess family needs, identify resources, make referrals, follow-up, and document client interactions in the UNIDAD and funder databases.

Conducts at least one home visit per month and on an as needed basis.

Reports all serious incidents involving clients and families to supervisor and completes the needed documentation within 24 hours.

Report on each initial client’s educational and social progress in detail through quarterly narratives and biweekly meetings.

Minimum Qualifications

Bachelor’s degree in Social Work or in a related Social Science field.

2-3 yrs. Case management exp. in providing supportive services in a community setting.

Knowledge of the Latino community and resources within the community and in the city at large.

Computer proficiency in Microsoft Office Suite

Preferred Qualifications

Bilingual (English/Spanish)

Exp. in working with TANF recepients
